The free assistive device rental service is only available within the Malacca area.
After your application is approved, the system will notify you via WhatsApp. Upon receiving the message, please reply and confirm your collection date, and collect the assistive device during the center’s opening hours.
If you do not reply or arrange a collection date within three days of receiving the notification, the system will automatically cancel your reservation and offer the assistive device to the next applicant in need.
Please arrange your own transportation to collect the device. If you book a courier service (such as Lalamove), please inform the Tzu Chi Melaka Assistive Device Center volunteers in advance via WhatsApp so they can help confirm your reservation and coordinate the delivery time.
If you are assisting someone other than a relative with the application, please provide both your information and the applicant’s information in the form.
During use, please monitor the assistive device for any difficulty, malfunction, or damage. If you notice any issues, stop using the device immediately to avoid injury and contact our center.
