
Eligibility:
- Citizenship: Malaysian citizens above 18 from the B40 group, residing in Melaka. Citizenship and age will be verified using the original Malaysian National Identity Card (MyKad). Applicants must upload an image of their MyKad.
- Residency: Residency will be confirmed using the address on the MyKad or utility bills. If the residential address differs from the MyKad address, applicants must upload an image of a utility bill (water or electricity) showing the residential address. Provide only ONE residential address.
- Change of Residential Address: Please notify us promptly of any change in your residential address by sending a WhatsApp message to 010-8821829.
Household Principle:
- Single Applicant: Only one person per household can apply and receive food items. A household is defined as individuals living at the same address.
- Family ID: Two individuals with the same address (e.g., spouses, parent and child) may submit separate applications and indicate their wish to apply for a Family ID. Upon approval, they will be linked with a Family ID. Either one can register for an appointment and collect food items if the appointment is successful.
Contact Information:
- Mobile Number: Applicants must provide ONE mobile phone number with WhatsApp capability for communication. Failure to meet this requirement will result in the inability to submit the application, lack of contact, or non-approval of the application.
- Change of Mobile Number: Please notify us promptly of any change in your mobile number by sending a WhatsApp message to 010-8821829.
